The Rehabilitation Hospital is a 20 bed comprehensive medical rehabilitation unit. Our mission is to deliver compassionate, quality, interdisciplinary rehabilitative care, including 24 hour comprehensive nursing. Rehab Nursing provides patient and family education, comprehensive rehabilitative nursing care, and reinforces skills learned in therapy to help patients return to their maximum level of independence.
Job Summary: The Case Manager works to improve patient outcomes by coordinating and facilitating patient care across an interdisciplinary care team while ensuring compliance with associated regulations and requirements and to ensure the care plan is revised as needed throughout the patient’s rehab course. The Case Manager works closely with the rehab Social Worker to ensure a seamless transition for the patient and caregiver at discharge. All duties and responsibilities are to be carried out with empathy and compassion for patients and their caregivers, as well as coworkers.
Minimum Requirements: Graduate from a school of nursing. Current license with North Carolina State Board of Nursing or Compact License recognized by NC State Board of Nursing. BSN preferred. Experience in an inpatient rehabilitation environment preferred. Two to three years of experience in hospitals, nursing homes, or community agencies. Have and maintain a valid license to operate a motor vehicle in the State of North Carolina. The staff member must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on his/her assigned unit. The individual must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient’s status and interpret the appropriate information needed to identify each patient’s requirements relative to his/her age-specific needs and to provide the care needed as described in the unit’s departmental policies and procedures.
