Patient Navigator, Cancer Center Support Services
Job Summary: The Oncology Patient Navigator functions on the multi-disciplinary team as an advocate, interpreter, educator, and counselor for oncology patients. She/he is responsible for facilitating the provision of quality and comprehensive services to all patients with an oncology diagnosis. This will be accomplished through coordination of care throughout the continuum in collaboration with the multi-disciplinary team. The Navigator will serve as a clinical resource with expertise in hematology/oncology care management and will serve as a liaison throughout the facility and in the community regarding services provided for this unique patient population. He/she will provide expert nursing care, which includes: direct clinical practice, consultation, education, and research.

Minimum Requirements:
- Graduate of an approved school of nursing. Bachelor's degree in nursing preferred.
- Oncology Nursing Certification preferred.
- Minimum of five years acute care nursing experiences with two years of experience in the area of specialization.
- Experience in provision of care in area of specialization performed in outpatient/home or other community setting preferred. Ability to think and plan independently and to work in an unstructured environment.
